Our Community Health Survey Will be Closing Soon!
If you are a resident of Lane or Douglas County and 18 or over, we would truly appreciate it if you could take this Community Health Equity Needs Survey in English or Spanish. This information will be used by a community-led group called RISE of the Umpqua and Willamette Valleys to help address health needs by working with local organizations and to inform laws and changes within the Oregon State Legislature. RISE of the Umpqua and Willamette Valleys will also be sharing the information we get back to the communities who participate in the assessment by creating a report, infographics, and presentations.

We are also doing a prize drawing for people who put in their email or phone number at the end of the survey! This information will only be used to contact you should you win the drawing and will be destroyed after the drawing is finished. The survey should take about 10 minutes to finish. Please share with your friends and family!
